I generated the PDF via iBooks Author; I haven’t explored how to do that. If you want the iBooks author template, I can get that for you (I need to do some cleanup).
And in a frustrating twist of fate, my iBooks Author is beginning to flake out. I suspect it is related to the bad image that was used to create my installation AND my root level shenanigans AND a compromised/corrupt font.
