Good stuff (and now a nice fresh bookmark, thank you), but not quite the one I was hoping to track down.
I have a print of this one (I keep it with my D&D character), it was 8 pages with a “Player Agenda” and “Player Principles” section (like the GM section in the main rulebook).
I was hoping to share the link with players in an online game that I run.

I put it together, but pulled from several writers far more brilliant than I. I mentioned them in the original post, and will try to find that to credit them again.
