Do you get +1 forward for each question answered, as used appropriately? Do you get +1 forward just once, which expires on use, but can only be used in relation to the answer?

The latter. Forward is one use, the first time it comes into play. Ongoing is the mechanic for a bonus that can be used more than once.
The last of the three? My initial impression was that it was the second one. +1 forward for each answer. Its interesting because I noticed that in AW read a sitch gives you +1 whenever you act on the answers, which is essentially ongoing, and the wording in DW is a bit more vague. In AW read a sitch is therefore a lot more of a game changing move. I wonder if there was an explicit reason to tone it down in DW?
