I had and lost a nice one page, three column questionnaire and cant find it again.
Anyone have things like this they are willing to share?

The questions should probably be specific to the world that comes up during character creation. You can have a list of generic ones, but “What is your town like?” is always going to be weaker than “You said you left Pendington as a child? Why? Who did you leave with?”
