Well, what is the move intended to do? Outright kill someone? What are the things that can go wrong while someone is attempting to do this?

When you wrap a garrote around the neck of someone who is unaware of your presence, roll +STR. On a hit you deal your damage and the target can not use its voice. On a 7-9 choose 1. While holding on take -1 ongoing to do anything else.
The target fights back, dealing its damage to you.
The target creates some noise, drawing unwanted attention.
The target starts to slip from your grip, take -1 ongoing to continue to strangle them.

I got ninja-ed by Delos Adamski. I like parts of that move, but I don’t think you need to say “While holding on take -1 ongoing to do anything else.” The Assassin is already using both hands to hold the garrote, right? So they are fictionally prevented from doing much else. Including the mechanical penalty seems redundant to me.
I don’t know if I would have the move do class damage. I mean, it’s a garrote. Around their neck. Hit points aren’t going to do jack.
Maybe I would do something like this:
When you wrap a garrote around the neck of someone who is unaware of your presence, roll +STR. On a 10+, choose two. On a 7-9, choose one:
– the target doesn’t make any noise;
– the target dies quickly;
– the target doesn’t fight back.

Not really. I have my own ideas but I don’t want to interfere 😉
Target silent and killing quickly are defintely two very good options.

My advice to you is to keep it simple, and start by thinking what the move does in the fiction. Don’t make something overly complicated unless you really need the extra rules to wrap around it.
I suggest naming it “Is Wayne Brady Gonna Have To?”
